Form 1 is a self-declaration/application form completed by DL applicants between the ages of 17 and 40. Form 1A is completed by DL applicants over the age of 40.
Candidates above 40 years of age should submit self-declaration form 1A.
You need to attach a valid identity proof, a valid address proof and birth certificate along with self-declaration form 1.
