Where to Obtain the Passport Application Form

Passport Form

Passport form can either be obtained online or offline:

  1. Online: The passport seva portal at https://www.passportindia.gov.in/ provides access to online passport application forms that can be completed in one of two ways. One method involves downloading the electronic form, filling it out offline, and uploading it later at the provided URL (check the online application process). 
  1. Offline: The second method involves filling out the electronic form offline and uploading it later at the site provided.  Additionally, there are two ways to access offline passport applications. 

Getting your passport form online is easier, quicker and more convenient than getting it offline. It should also be noted that going the offline route is only possible for new or fresh passport application forms. This is because the abovementioned centers (except PSKs) do not process any other type of application.

Where to Submit Your Indian Passport Application

Depending on the application method, passport application forms can be completed and submitted either online or offline. Print forms are submitted offline, while online forms are submitted online. On the passport website, online applications are made. 

The PSK and other collection facilities are where you can submit documents offline. Depending on how the forms are filed, the amount of the passport application fee and how the documents must be presented may also change.

e-Form for Passport Application

You either fill them out and submit them online or download them first, fill them offline then upload them later. You can download the following e-forms from the passport website.

  1. Form for new or fresh passport application
  2. Form for renewal passport application
  3. Forms (individual) for police clearance certificate, identity certificate and surrender certificate application

For applications processed at PSKs or regional passport offices (RPOs), e-forms have to be submitted online i.e. Printed versions of the same will not be accepted at these centers. Also, applicants must have at least version 9 of the acrobat reader to open any of these e-forms.

How to Print the Passport Application Form

The passport portal offers links to forms that may be downloaded and printed even though the e-forms can only be submitted online. They can be completed offline by handwriting in the information and physically turned in at the appropriate passport centre. These forms must be printed back-to-back on a4-sized paper.
The following printable forms are available for download from the passport website -

  1. Main form for new or fresh/ renewal or reissue passport application
  2. Supplementary form for new or fresh/ renewal or reissue passport application
  3. Instructions booklet for the abovementioned main and supplementary forms
  4. Forms (individual) for police clearance certificate, identity certificate and surrender certificate
  5. Form for loc permit

As with the e-forms the applicants must have at least version 9 of the acrobat reader to open these forms.

Annexures and Affidavits

Affidavits, particular certifications, declarations, and particular letters, among others, are submitted with passport applications as annexures when necessary. There are several different annexures and/or affidavit kinds that can be accessed, downloaded, and printed, including -

  1. Annexure A : This is an affidavit for submission by illiterate applicants supporting their birth date.
  2. Annexure B : All employees of the central or state governments, statutory bodies, and public sector organisations are required to provide this identity certificate. This also applies to such employees' wives and minor children (those under the age of 18).
  3. Annexure C : This is an affidavit for the submission of a passport application by the mother or father of a minor kid in a situation where the parents are not legally divorced but are living apart.
  4. Annexure D : This is an affidavit for submission by a lady applying to change her name in the passport post marriage.
  5. Annexure E : This is an affidavit for submission in case of application for change in name or deed or sworn affidavit.
  6. Annexure F : This is a verification certificate for passports applied for on tatkal basis. Passport authorities may verify this certificate for authenticity.
  7. Annexure G : this is a declaration for submission by a minor's parent or guardian for a minor's passport. This is in cases where only one parent has given consent for the minor to obtain a passport.
  8. Annexure H : This is a declaration for submission by a minor's parents or guardians for a minor's passport.
  9. Annexure I : This is the standard affidavit in which the applicant states his\ her name, his\ her parents' and spouse's name (if any). The applicant also states his\ her residential address, and that he\ she is an indian citizen and free of criminal record among other details.
  10. Annexure J : This sample verification certificate must be enclosed with the standardised affidavit previously indicated (annexure i). Directors, owners, or partners of its member firms get it as a gift from the chairs of top business associations.
  11. Annexure K :  This sample verification certificate must be enclosed with the standardised affidavit previously indicated (annexure i). Directors, owners, or partners of its member firms get it as a gift from the chairs of top business associations. 
  12. Annexure l : this is a specimen affidavit for submission to apply for a passport in place of a damaged or lost passport.
  13. Annexure M : this is a no objection certificate issued by a particular ministry or department or office stating that no objection to the applicant getting the passport.
  14. Annexure N : This is a letter of advance notification for submission by a candidate who works for the government, a public organisation, or a statutory authority. The letter is completed to let the employer know that the applicant is applying for a passport.
  15. Authority letter :This is a letter of authorization that a passport applicant fills out to allow someone other than themselves to submit their application. It can also be used to give someone else permission to pick up the passport and any necessary paperwork.

Undertaking GSR 570 (e)

This statement of undertaking includes the applicant's name, parents' names, and spouse's name (if any), and is submitted on plain paper. Along with the pertinent passport file number or application reference number (ARN), it also contains the applicant's current living address.

  • Can a PCC or a Police Clearance Certificate be issued for more than one country through a single application form?

    No, a Police Clearance Certificate can be issued for a single country through a single application form.

  • In case an individual has written his\ her full name in the 'Given Name' field but has left the 'Surname' field blank, what is the process to be followed to add a surname?

    If the passport book and the application form are both incorrect, they must be returned for the necessary verification. Depending on the decision of the passport officers or assistant passport officers, further fees, if any, may need to be paid. If a factual deception is discovered, sanctions may be applied. As a result, the applicant must be very attentive while supplying information for his or her passport and should make sure that the material is accurate and free of typos. The applicant will need to submit an application for a new passport if a mistake was made.

  • Is it mandatory for applicants to paste a photograph in the printed copy of an online Official or Diplomatic Passport application form?

    It is true that applicants must attach two passport-sized coloured photos (4.5 cm x 3.5 cm), shot against a white backdrop, to the printed version of the online-filled form. The first picture must be pasted on the first page of the application and cannot have a stamp or a signature on it. The third page of the application form must contain the second photo. The Head of Office's signature and office stamp must be put across this photograph.

  • Will original documents have to be produced or will certified copies be enough?

    Self-attested copies of necessary documents will have to be attached to the application form and originals must be provided at the counter. The original documents will be given back after verification.

  • When will Annexure B, M and N have to be given and are these documents mandatory?'

    Employees of Public Sector Government Statutory bodies who wish to obtain a new passport must submit an application form and either an original Identity Certificate as per Annexure B, a NOC or No Objection Certificate as per Annexure M, or a PI, or Prior Intimation Letter, as per Annexure N. If Annexure B is provided, a passport will be issued immediately; if Annexure M is presented, a passport will be issued after police verification. The passport will be issued after receiving Annexure N and undergoing pre-police verification. 

  • In case a government employee wants to apply for major changes in their passport, what documents will they have to be submitted?

    Gazette Notification changing name, Fresh ID Certificate in the new name, Deed poll\sworn affidavit as per Annexure E

  • Is No Police Verification(PV) possible in case Government\ PSU\ Statutory Body employees changes their name?

    Yes, it is possible and pre-PV will be done. Only in cases of Tatkaal applications, post police verification will be done.

  • Can dependents of Government\ PSU\ Statutory Body employees apply for a passport through the normal procedure without the need for affidavits?

    If the dependents of Government\ PSU\ Statutory Body employees submit the original Identity Certificate as per Annexure 'A', passports will be issued to them without police verification.

  • Will applicants born prior to 26 January 1989 have to submit a Birth Certificate?

    Yes, these applicants will have to compulsorily submit a Birth Certificate that has been issued by a Municipal Authority or office that is authorised to issue a Birth Certificate.

  • Will a Birth Certificate have to be submitted in case of reissue of Passport?

    If the old passport has been lost, destroyed, or if the applicant wants to update information in the passport, proof of date of birth will be required when applying for a new passport.

  • What are the documents needed for changing the date of birth in the individual's passport?

    Applicants will have to apply for a reissue of passport and get the necessary change done in personal particulars.

  • What additional documents will have to be submitted for change in gender or change in parent's name in the passport?

    A person must submit an application for a new passport and make the required adjustments to their personal information.

  • Can an individual use Bank Statement or Passbook as proof of address?

    No, A Bank Statement cannot be used as proof of address.
