How to fill Passport Application form

1. Fill all details in the form in capitals, with one letter in each box.
2. Avoid using fancy fonts and use standard fonts only.
3. Use a blue or black ballpoint pen and not a pencil or ink pen.
4. Mark a cross in the boxes where you have to choose one or more options as your answer and leave the other option(s) blank. Do not dot or tick mark.
5. Avoid touching boundaries of the box while writing.
6. While writing, leave one box empty after completing each word.
7. Do not write anything outside the box.
8. Do not overwrite. Just strike out the character in case of a mistake and continue to write in the next box.
9. Fill in only those details that are required and leave the remaining blank. Don't strike out or mark as not applicable or 'NA' if a field doesn't apply to you.
10. Don't fold or dirty or smudge the form.

Passport Photograph Requirements

If the form is to be turned in at a PSK or a mini-PSK, do not paste a picture. If the form is to be submitted at another facility, such as a DPC, SPC, or CSC, you must include a photo with the following specifications.

1. Unsigned photograph (don't sign over it once pasted).
2. Only one photograph to be pasted, fitting in the space provided, neither bigger nor smaller.
3. Latest photograph in color printed.
4. Frontal view of candidate, head centred and both ears and facial edges visible, eyes open, neutral facial expression.
5. White background, dark outfit/dress (uniforms not permitted).
6. Dimensions - 4.5 centimetres X 3.5 centimetres.
7. Good quality paper to be used for the photo, computer prints are not permitted, damaged/torn photos are not permitted.
8. Sign under the passport in the box provided. In the case of minor applicants, the signature/thumb impression will have to be of the minor only. Parents/guardians should not sign, under the photo, on behalf of the minor. Ensure your signature/thumb impression is well within the box to be accurately scanned.
9. Photographs that are cut from group photographs will not be accepted.
10. Candidate's hair should not cover the eyes.
11. Head covering is allowed only for religious reasons.
12. Photographs with distracting shadows on the candidate's face or the background will not be accepted.
13. Glares on eyeglasses must be avoided by tilting the head slightly upward or downward.
