- On the appointment date, visit the PSK with the documents.

- Duly filled application form
- Original old passport along with the self-attested photocopies of the first and last two pages of passport.
- Self-attested copy of the Divorce Certificate
- Age Proof
- A copy of a death certificate of spouse (in case of the spouse’s death)
- Sworn Affidavit or Deed Poll as per Annexure ‘E’
- Identity proof
- Address proof
