All information pertaining to the said schemes has to be furnished in Form 2. For instance, an individual can nominate one or more persons of his or her family to get the provident fund proceeds upon death.

Under Employee Provident Fund Scheme 1952, ‘family’  includes husband/wife, offspring (married/unmarried, adopted), in addition to dependant parents. Under Employees Pension Scheme 1995, the family includes husband/wife and children.

If an employee wishes to change his or her nominees under Employees Pension Scheme 1995, he or she should send a revised Form 2 via the employer.

Types of EPF Nomination Forms

There are three types of nomination forms as listed below:

  1. PF Form 2: This is a common form (pension and PF schemes) used for payment of fund amount to the nominees.
  2. PF Form 11: The Employees' Family Pension Fund Scheme and Provident Fund are both in effect at the factory or place where the member declares employment.
  3. PF Form 9: This form is a declaration by an individual taking up work in a factory/ establishment where the Employees' Pension Scheme is applicable.

Required Information to Fill PF Form 2

PF Form 2 is a nomination and declaration form under the Employees Pension and PF schemes (paragraph 18 of Employees Pension Scheme 1995 and the paragraph 33 and 61 (1) of Employees Provident Fund Scheme 1952).

EPF Form 2 requires a member to fill in the following details:

  1. Name of the member
  2. Father/husband’s name
  3. Date of birth
  4. Permanent address
  5. Name of the nominee
  6. Complete address of the nominee
  7. Member’s relationship with the nominee
  8. Date of birth of the nominee
  9. Share of the PF amount to be paid to each nominee
  10. In case the nominee is a minor, the member has to furnish details of the guardian who will receive the PF amount
  11. In case the member does not have any dependent parents or family as mentioned under para 2 (g) of Employees Provident Fund Scheme 1952, he or she should strike out the sections which are not applicable
  12. Signature of the subscriber
  13. Details of family members who will receive the children/widow pension when the member dies. The member has to mention the name, age and address of the family members. The member should also mention the relationship with the nominees.
  14. Name and address of the establishment/factory
  15. Signature of employer/ authorised officer

Required Information to Fill PF Form 11

EPF Form 11 is a nomination and declaration form under para 24 of the Employees Pension Scheme (EPS), 1995 and para 34 and 57 of the Employees Provident Fund Scheme 1952. PF Form 11 requires a member to furnish some details as listed below:

  1. Name of the member
  2. Date of birth of the member
  3. Father/Husband’s name
  4. Mobile Number
  5. Email ID
  6. Individual has to mention if he or she was a member of Employees Provident Fund Scheme, 1952 and Employees’ Pension scheme 1995. If yes, then the individual has to fill out details regarding his or her previous employment such as listed below:
    1. UAN or previous provident fund account ID
    2. Date of exit
    3. Previous scheme certificate number
    4. Previous pension payment order
  1. International workers have to specify the following:
    1. Country of origin
    2. Passport number
    3. Passport validity
    4. Educational qualification
    5. Marital status
    6. Any disabilities (visual, locomotive, hearing)
  2. KYC details to be included are as follows:
    1. Bank account number with IFSC code (mandatory)
    2. Aadhar
    3. PAN
    4. Passport
    5. Driving License/voter-id.html
    6. Voter ID
    7. ESIC card
    8. Ration Card

All the above mentioned documents must be self-attested

  1. Declaration by the present employer
  2. Signature of the employer along with establishment’s seal

Required Information to Fill PF Form 9

Employees Pension Scheme, 1995 (paragraph 34) and Employees Provident Fund Scheme, 1952 are both declared on EPF Form 9. (para 34 and 57). This form serves as a declaration that a member has begun working for an organization that is subject to both the Employees' Pension Scheme of 1995 and the Employees' Provident Fund Scheme of 1952. Form 9 requires a member to furnish the following details:

  1. Name of the member
  2. Date of birth of the member
  3. Father/Husband’s name
  4. Mobile Number
  5. Email ID
  6. Individual has to mention if he or she was a member of Employees Provident Fund Scheme, 1952 and Employees’ Pension scheme 1995. If yes, then the individual has to fill out details regarding his or her previous employment such as listed below:

Pre-requisites for EPF E-Nomination 

Here are the pre-requisites for EPF E-Nomination: 

  1. Make sure your gateway profile picture is up to date. You won't be able to edit your EPF nominee online if this happens. The same holds true for a person's marital status, current and previous addresses, and birthdate.
  2. Your name, gender, and date of birth mentioned on the EPF records should be same as that on your Aadhaar card.  
  3. For EPF E-Nomination, you need to have important information about the nominee such as their name, photograph, date of birth, and Aadhaar card number.  
  4. Your Aadhaar card and mobile number should be linked.  
  5. Your siblings and sisters are not considered to be family members in accordance with PF law. This implies that you cannot name them as your PF beneficiary if you select "Yes" for the "Having Family" option. You must select "No" for "Having Family" if you want to nominate your siblings because you are not married.

Facts about EPF E-Nomination 

Here are some facts about EPF E-Nomination: 

  1. A PF member can nominate more than one family member.  
  2. An unmarried member is allowed to nominate any individual for the PF regardless of their relation.  
  3. A member who does not have a spouse or children may nominate any individual for pension contribution. 
  4. If there is no spouse or children, only the pension nomination link will be displayed, and the member can nominate any individual. 
  1. Any married member with a spouse and children must nominate them. For the pension fund, spouse and children are regarded as family members. 
  2. PF members must make sure to update the nomination if any family members change as a result of the birth or death of a nominated person. Family members will benefit from receiving their dues in this way.
  3. Any earlier nomination made by the members for their PF and EPS account becomes invalid once they marry, according to the EPF scheme rules. 
  4. After getting married, a member who previously submitted an EPFO e-nomination as single must do so again because the prior nomination is now invalid.
  5. A PF member may modify their EPFO e-nomination at any time. The previous nomination will be replaced with the e-signing of the new one.  
  1. A fresh nomination can be filed and e-signed by a member. However, the previously e-signed nomination cannot be edited. 

How to update your photograph

Follow the steps mentioned below to update your photograph -  

Step 1: Click on the link https://www.epfindia.gov.in/site_en/index.php

Step 2: Under the 'Services' tab, click on 'For Employees'.

Step 3: Click on the 'Member UAN/Online service (OCS/OTCP)'

Step 4:  You will be directed to the link https://unifiedportal-mem.epfindia.gov.in/memberinterface/.

Step 5:  Sign in using your UAN number, password, and captcha.

Step 6:  Once you have logged in, click on 'View' and select the 'Profile' option from the drop-down.

Step 7: You can see all the information by choosing the "Profile" option. Select "Change Photo" from the menu on the left to upload a new photo to replace the one you currently have.

Step 8: Before uploading the photograph, you can preview it and if you are satisfied, click on 'Upload Photograph'.

How to update your Address

Follow the steps mentioned below to update your address in the PF portal -  

Step 1: Click on the link https://www.epfindia.gov.in/site_en/index.php

Step 2: Under the 'Services' tab, click on For Employees'.

Step 3: Click on the 'Member UAN/Online service (OCS/OTCP)'.

Step 4: You will be directed to the link https://unifiedportal-mem.epfindia.gov.in/memberinterface/.

Step 5: Sign in using your UAN number, password, and captcha.

Step 6: Once you have logged in, click on 'View' and select the 'Profile' option from the drop-down.

Step 7: On selecting the 'Profile' option, you will be able to view all the details. In the details, you will be able to view both your permanent and current addresses. Click on 'edit' for the address you want to update. Enter your address.

Step 8: Once you have entered the new address, click on 'Update'.

Steps to file your E-Nomination online

Follow the steps mentioned below to file your e-Nomination online -  

Step 1: Click on the link https://www.epfindia.gov.in/site_en/index.php

Step 2:  Under the 'Services' tab, click on For Employees'.

Step 3:  Click on the 'Member UAN/Online service (OCS/OTCP)'.

Step 4:  You will be directed to the link https://unifiedportal-mem.epfindia.gov.in/memberinterface/.

Step 5:  Sign in using your UAN number, password, and captcha.

Step 6:  Once you log in, click on 'Manage' and select the 'E-nomination' option from the drop-down option.

Step 7:  A new page will open, where you will have to click on 'Family Declaration Column'.

Step 8:  Select either 'Yes' or 'No' depending on whether you have a family or not.

Step 9:  Under the 'Family Details' option, add the details of your nominee.

Step 10:  In the next option, enter the details of the nominee such as their Aadhaar details, gender, date of birth, bank details, etc.

Step 11:  If you want you can add more than one nominee too. If you want to add more nominees, click on 'Add row' option and provide their details.

Step 12:  For one nominee, you can add 100%. For multiple nominees, you can break down the percentage of amount they will receive.

Step 13:  Click on 'Save EPF Nomination'. Click on 'E-Sign', following which a pop-up will appear 'Do you wish to proceed for e-sign?'. Click on 'Proceed'.

Step 14:  You will be directed to the 'Demographic Authentication page'.

Step 15:  Enter your virtual ID, tick the consent box, and click on 'Verify'.

Step 16:  The E-hastakshar page will open where you will have to tick the box.

Step 17:  In the next step, enter your Aadhaar number or Virtual ID and click on 'Get OTP'.

Step 18:  A One-Time Password (OTP) will be sent to your registered mobile number. Enter the OTP, give your consent, and click on the 'Submit' button.

Step 19:  You have successfully filed your e-Nomination online.

It is advised that you finish the e-Nomination process right away to ensure that your family members have a smooth claim process.

FAQs on EPF Nomination Form & PF Declaration Form

  • What is the purpose of submitting PF nominations?

    If a PF member passes away before collecting the pension and there are no eligible family members, the nominee will receive the pension amount.

  • Is e-nomination compulsory for filing advance claims?

    No, e-nomination is not compulsory for filing advance claims.

  • According to PF laws, who comprises a family?

      Your main family comprises your spouse, children, and dependent parents under the PF laws. In terms of your PF nomination, your siblings are not considered family members. 

  • Who will receive the PF money in the absence of a valid nomination with no family member?

    In such circumstances, the PF money will be payable to dependent parents. The PF amount is given to the father first, followed by the mother.

  • Can unmarried individuals nominate someone not related to them as their PF nominee?

      Yes, however, once a 'family' is formed, the nomination becomes invalid, and the advantages under EPS-1995 are passed on to the spouse and children, if any. 

  • If there is no nomination, how will the PF money of a deceased member be paid?

      According to paragraph 70 (ii) of the EPF scheme of 1952, PF money must be paid to family members in equal shares. It is paid to people who are legally entitled to it in case there are no eligible family members. 

  • Is it possible to nominate someone other than my family as my PF beneficiary?

    Yes, you can nominate someone other than your family as your PF beneficiary. 

  • If a previously unmarried member marries, will the earlier nomination be valid?

    No, in such a circumstance, they will have to file a fresh nomination.
