EPF Form 2 - Nomination Form for Provident Fund & Pension Scheme

The EPF Form 2 asks for a few details to process your application. This information includes information like name, marital status, address, and date of birth.

Part A (EPF proceeds)

The Part A of EPF Form 2 deals with nominating people to receive the account EPF balance in the account holder’s EPF account in the event of his or her death.

In the tabular column given in the form, you need to fill in details of the nominee(s) such as name, address, relationship with member, age, amount to be paid to each nominee, and guardian details in case the nominee is a minor.

The section has to be signed or a thumb impression has to be made at the end of the section.

Part B (EPS proceeds)

The EPS proceeds of a member’s PF account is also disbursed to eligible nominees. Once again, you should fill in details of nominee(s) such as name, address, relationship, age, amount to be distributed to each nominee and guardian details if the nominee is a minor. The section has to be signed or a thumb impression has to be made at the end of the section.

This table requires you to furnish details of the family member(s) eligible to receive the children/widow pension. You need to fill information for name and address of family member as well as age and relationship with the nominee(s). The section has to be signed or a thumb impression has to be made at the end of the section.

Guidelines for nominating people

Under EPF scheme

A member of EPF who is married with/without parents who are dependents can nominate one or more persons in his family as per:

  1. If the member is a male, then his wife, children, dependent parents and deceased son’s widow and children can be nominated.
  2. If the member is a female, then her husband, children, dependent parents, husband’s dependent parents, deceased son’s widow and children can be nominated.

Under EPS scheme

A member of the Family Pension Scheme can nominate a family member to receive proceeds on his/her demise. Family is defined as:

  1. Wife of a male member
  2. Husband of a female member
  3. Minor sons and unmarried daughters

Sons and daughters can also include children adopted legally before demise in service.

In case a member does not have any family, then the monthly family pension will not be paid to anyone. However, the life assurance benefit will be given to nominees entitled to receive the member’s PF accumulations.

Details to be mentioned in EPF for nomination

Given below is the list of details that you will have to provide for EPF nomination:

  4. Relationship with the member
  5. Subscriber's signature or thumb impression

Details to be mentioned in EPF for nomination for monthly widow pension

Details to be provided in order to receive a certificate from the Employer

  1. Employee's name
  2. Signature of the authorised officer

How to fill e-Nomination online

Given below are the steps to fill e-Nomination online:

  1. Visit the EPF website and log in to the portal using your UAN and password.
  2. Under the 'Manage' section, click on the "e-Nomination" option.
  3. Under 'Profile', enter 'Permanent Address' and 'Current Address' and click on 'Save'.
  4. Select whether you have a family or not.
  5. Enter the details of the members of your family as specified and required and click on 'Save Family Details'. If you want to add more nominees, select the 'Add Row' option.
  1. Select the nominees from the list and enter the total amount of share.
  2. Click on 'Save EPF Nomination'.
  3. The details of your nominee will be saved.
  4. Complete the 'e-sign' the nomination process. Select the "e-nomination" option listed under the 'Manage' section.
  5. Provide the virtual ID of your Aadhaar and click on 'Verify'.
  1. Provide your consent for Aadhaar's e-KYC services.
  2. Enter the details of your Aadhaar or Virtual ID again and click on 'Get OTP'. A one-time password (OTP) will be sent to your mobile number linked with Aadhaar card.
  3. Enter the OTP to successfully register your nomination with EPFO. Once the process is completed, no further physical documents will need to be submitted.

Things to remember while nominating members of your family

  1. You can change your nominees whenever you want to.
  1. It is recommended to have more than one nominee.
  2. If the subscriber has no family member, then they can nominate anyone.
  3. More than one family member can be the nominee with a defined amount to be received by each.
  4. Every individual who joins an EPF-qualified establishment is required to join the EPS provided their monthly salary is Rs.15,000 and above.
  5. Minimum service of 10 years in order for the employee to be entitled to a pension.
  1. If the subscriber has a family, they cannot nominate anyone outside the family.
  2. If an unmarried subscriber marries, they will have to file a fresh nomination.
