1. Enter addresses for the previous years using the available fields.
2. To add more records, click the 'add' button; added documents will appear at the top.
3. Note that you will be asked for your present address and addresses for the last five years. If you select 'nativity by birth', you do not need to complete this section.

Step 5: Submit the Form

1. Click the submit option to finalize your application. If you need to cancel, click 'Cancel'.

1. Upload the documents that are required according to the specified file type and size.
2. Uploaded documents will appear at the bottom. You can delete any file by using the cross icon.
3. Note that when you browse and upload a document, the 'upload' option will appear.

2. To save the acknowledgement, click the print receipt option.

1. After submission, your request will be saved as a draft.
